There seems to be a duplicate in the toolset as I came across a Worg that dropped one each of the new and old items.
The old one stacked with an old one I still had. All other Worgs have only dropped 1 instance of the new.

I wonder if this might be the case for all magic blood drops where, on rare occassions, I've noticed that all these creatures can drop two. Maybe it's under those circumstances where this will happen.
