Some monsters casting harm on themselves

I saw this once or twice, most recently on Nomad Clerics, they seem to cast harm on themselves to heal themselves... doesn't work.

It happens if a battle is starter, they lose health, and I go into hiding (combat ends).

Their health go down after the cast.

Edit: just realized, it's probably just a harm spell, not necessarily harm itself...
