Sometimes a mining node (possibly other nodes?) will break and become unable to respawn until a reset. What happens is at some point near or at the end of the HP, the node will appear to glitch out and fall through the floor but you retain a targeting reticle and will unleash one more flurry before it truly vanishes. Even if you stop attacking right away the node still becomes broken until a reset.
I've seen this happen across all area of the UD, don't play on the surface so can't comment there.
