Case of the Disappearing Shield
I had an adamantine tower shield equipped since inventory was full. Tick brings me to level up, I do the level up and the shield disappears entirely. Judging from the logs, when I finished the level up there was a message about AC not stacking. Checked around the area, shield did not drop.

You should NEVER have a full inventory when You carry a Shield or a Weapon on Your character.
Its a bug with base NWN and cannot be fixed.. there are countless of reports from people who have lost their Shields/Weapons this way and however we would like, we cannot do anything about this sadly.
It took me 3 shields to learn it by hard to always take care of spare space in my inventory.. even when I knew about this bug.
I fully sympathize with You, but we cannot do anything about it.
