Idk if it's meant to do this...but I don't think Shades likes females.

This happens to my shadowdancer shadow sometimes too but corrects itself after a while.

Mine sometimes get summoned without heads. Only the odd time though.

It's just a nwn display bug with adjusted dynamic models. Can't really fix that on my end, sadly, but it's rare.
