For now it's good enough - just pretend it's a trophy from some place nearby where the ear would be that looks something like an ear. The same goes for PCs who happen to not have any ears. Thanks! This is on my radar. It works for me sometimes but not always. Movement seems to trigger an aoo very consistent, but spell casts have mostly stopped invoking AOOs when they are turned off.
